Requesting Your Medical Records

Under Ontario’s Personal Health Information Protection Act (PHIPA), you have a right of access to your own personal health information.

Requests for access can be made to Humber River Health by completing the Consent Form or writing to us. Please note that we do not accept typed signatures. If you cannot print the document to sign it, please review this document: How to Sign a Consent Form Electronically.

Please include a copy of your government issued photo ID.

Provide as much detail as possible in regard to which records you are requesting. For example, “Lab results and radiology report from 01/01/20 ER visit”.

A minimum administrative fee of $30.00 shall apply to requests over 20 pages.

Once the consent form is completed, please mail, hand deliver, fax or e-mail it to:

Your request will be processed in the shortest time possible, although we have 30 days to respond.

If you are a patient’s Power of Attorney or Substitute Decision Maker, please include the related documents with your request.

You will be required to provide us with valid government issued photo identification or other proof of identity prior to receiving the personal health information.

If you are a healthcare provider, please fax consent forms directly to 416-242-1085.

Corrections to Your Record

If you believe that the personal health information in your medical record is inaccurate or incomplete, please submit a completed Request for Correction to Personal Health Information Form along with a copy of your government issued photo ID to:

F.A.Q.

Can you send my health information to my family physician?

We can only release information without written consent to a family physician if it is listed in the patient’s medical record at HRH. If you provided this information to us at your last visit to HRH, your family physician can call the Release of Information Department at 416-242-1000 ext. 82371.

If we have no mention of a family physician, we require a signed Consent Form to be faxed to 416-242-1085.

There is no fee to release information for medical purposes or continuing care of a patient.

How do I request a death certificate?

The hospital does not issue death certificates. A Medical Certificate of Death is completed by the hospital and is sent to the funeral home with the deceased. A Statement of Death is completed at the funeral home and the death is registered with the Office of the Registrar General at Service Ontario. To obtain a government issued death certificate, please contact Service Ontario at 1-800-461-2156 or through the Service Ontario website.
