The SCOPE Program at Humber River Health

The Seamless Care Optimizing the Patient Experience (SCOPE) Program launched at Humber River Health on January 12, 2021, which provided virtual interprofessional team support to family physicians through a single point of access in North West Toronto.
Below are various services provided by the SCOPE Program.
Internist On-Call
The Internist on-call provides the primary care provider with an immediate consultation and urgent assessment and management of their patient’s medical conditions. The Internists connect family physicians to acute medicine to better support complex patients in the community.
Home and Community Care Coordinator
The Home and Community Care Coordinator facilitates access to home and community resources for patients with complex or chronic care needs and/or existing Home and Community Care clients, thus improving integrated care in the community.
Nurse Navigator
The SCOPE Nurse Navigator provides general advice and assistance navigating health, community, and SCOPE services/resources for patients, which includes
- Searching for appropriate Health/OHT-based resources
- Communication between primary care and Health physicians
- Expedited patient care when possible
Diagnostic Imaging
The SCOPE Diagnostic Imaging Service answers questions about different tests and results, escalates urgent imaging requests, and can refer directly to the radiologist on-call.
