The Government of Ontario has a vision to build a connected, sustainable healthcare system centred on the needs of patients. Key to realizing this vision is the creation of Ontario Health Teams (OHTs).  Within this new model, health care providers will work as one coordinated team to make it easier for patients and our community to navigate the system and transition between healthcare providers.

As you may know, The North Western Toronto Ontario Health Team (NWT OHT) partners are a collaboration of healthcare providers that cover a broad scope of services including primary and acute care, rehabilitation, complex continuing care, mental health, long-term care and support services. The NWT OHT was 1 of 31 OHTs invited to submit a full application to the Ministry of Health which will identify our approach to providing integrated care and redesigning service delivery.

For your consideration:  On behalf of the NWT OHT, we are inviting our community partners, to complete a letter of support that will help strengthen our OHT application for the Ministry of Health.  We have attached a Letter of Support – Template for your consideration. If you are willing and able to provide us with a letter of support, could you please submit it by Sept. 30, 2019 to our Corporate Communications & Public Affairs Department.
